Bereavement Ministry
Perpetual Mass/Prayer Enrollment is offered by the Monks at the Monastery of the Holy Spirit in Conyers, Georgia.
The Knights (Tom Carriere) will handle the arrangements when there is a serious illness or death in a Knight’s immediate family and the family desires that a Perpetual Mass Card be sent.
Complete the Enrollment Form and it will be emailed to Tom Carriere. Tom will then make all the arrangements.

The information on
the Enrollment Form should be completed and a
picture selected for the Perpetual Mass Card.
There is no charge for
the card.
The form (or the same information) can either be sent via
submitting the form, fax (at the
